Form Thirty-Seven
Bureau's Most Wanted File
Knows Too Much to Survive
Aspects refreshed Jul 08, 2026
Form Thirty-Seven was created during the Bureau's founding in 1954 as the 'Original Truth' document - the master file meant to preserve uncensored player histories for eternal reference. When the Bureau later implemented its redaction protocols, three copies of Form Thirty-Seven were secretly preserved in the Deep Archive against protocol, each guarded by a different internal faction who suspected the others of manipulating the truth. These three copies have never been reunited, and the Bureau officially claims Form Thirty-Seven was destroyed.
Form Thirty-Seven manifests differently to each viewer, revealing only what they're personally cleared to see - making it simultaneously the most revealing and most frustrating document in existence. It cannot be photographed, photocopied, or digitally transmitted; any attempt results in blank pages or corrupted files. The document glows with a faint orange luminescence matching the series accent color, and the form number '37' in the header appears to shift position when observed indirectly. Reading Form Thirty-Seven aloud in the presence of other players triggers an automatic Bureau audit of everyone within earshot.
Form Thirty-Seven serves as the ultimate intelligence weapon and greatest liability in the Containment Zone. Players who possess it can demand to see any opponent's complete, unredacted file, cutting through redactions, Lost Dossiers, and White Out corrections to reveal the raw truth. However, the Bureau will pursue holders to the ends of the Containment Zone, and other players immediately become suspicious of anyone suspected of carrying it - because if you can see their truth, what can you do with that information?

Commentary from Flippy (your trapped narrator)
The anomaly known as Form Thirty-Seven continues its unauthorized migration through the Grey Matter sector. Christian Phelps shot a 51—four strokes under the field average—and leaped from the Congregation's #18 straight into The Elect at #3, just two rungs from the Trinity. His 860-rated round sits 11 points below his PDGA rating, which is technically 'slightly off,' but the Bureau's metrics don't account for the document's reality-warping glow. stamps form Redacted. Moving on. The Visitor may need to recalibrate the throne.

Form Thirty-Seven has slipped the archive for a Grey Matter side quest. Christian Phelps is the new custodian at Creekside Park, carrying a document that glows orange and shifts numbers when you blink. It’s a spinoff arc, folks—just a local league detour where the Bureau’s redactions can’t reach the chains.
